GENERAL SUMMARY

The Behavioral Health Liaison gathers patient information and works in collaboration with the attending provider and the clinical team to facilitate patient discharge/transfer to next most appropriate level of care.

ESSENTIAL RESPONSIBILITIES

  • Interviews patients and gathers information relevant to the development of a plan of care for disposition.
  • Contacts behavioral health and social service agencies to gather information, after obtaining patient/legal guardian consent.
  • Provides a summary of findings to attending provider and documents same in patientrsquo;s medical record.
  • Collaborates with attending provider and clinical team to develop a plan of care for disposition and documents consultation in medical record.
  • Facilitates patient discharge/transfer to next most appropriate level of care

JOB REQUIREMENTS

Education required: Masterrsquo;s degree in social work, counseling or related social science field.

Experience required: Three years in a behavioral health program or facility.

Preferred: Care coordination, utilization/case management or discharge planning experience. LCSW, LPC, LSW; mental health/chemical dependence certification.
