Short Description

Provides case management services for 50 HIV positive individuals.

Provides emergent case management services for EIP patients as needed.

Complete case management assessments and develop client service care plans.

Monitor and evaluate service care plans quarterly.

Assess all EIP clients requesting or referred for CM services and identify immediate/urgent
needs and take appropriate action.

Will meet objectives for the provision of referrals and linkages to support services.

HIV Counseling and Testing as needed.

CM services provided in accordance with AACO and the NJDHSS standards for HIV
medical case management services.

Bi-Lingual English/Spanish a plus!


Experience Required

1 year or more experience in the field of Case Management or Social Services.

Experience in the field of HIV preferred.

Extensive knowledge of available financial and social service resources in southern New Jersey.

Excellent interpersonal skills.


Education Requirements

Bachelor's degree required


License/Certification Requirements

AACO or NJ HIV Case Management Certification preferred


Special Requirements

Bi-Lingual English/Spanish preferred
