Facilitates patient identification, screening, and enrollment for program eligibility. Monitors, manages, and addresses clinical pools and documents events in patients' digital health record, escalates as appropriate. Support vital sign alerts. Collaborates with post-acute networks, community organizations, and vendors to facilitate care plan. Facilitates scheduling of imaging and laboratory testing. Help support care coordination, patient care needs, and navigation. Performs prior authorizations, medication refill requests, and pharmacy calls. Communicates with patients on test results at the providers discretion. Collaborates with health care team. Reinforces providers post visit instructions and serves as a resource for patients and caregivers as it relates to their scope of practice. Patient and caregiver education. Maintains privacy and confidentiality for all patients. Regularly attends in-services to further develop position and department. Performs all related duties or special projects as assigned/required.
