Job Summary:

This role is responsible for working collaboratively with members of the Quality & Health Equity Team and HNS to implement, monitor and evaluate Horizon clinical opportunities to develop enterprise and community health initiatives. These initiatives will be aimed at addressing Health Disparities and advancing awareness and education about social determinants of health to develop innovative strategies to advance health equity in health care access and delivery for our members. The Director, Special Health Disparity Initiatives leads the planning and execution of engagement, outreach actions and activities with community and faith-based organizations, allies, and partner organizations.
The Director, Special Health Disparity Initiatives is a member of the Quality and Health Equity Leadership team and will be responsible for providing input and brining awareness to health disparities, and opportunities for outreach strategies that may influence other functional areas across the organization and external communities.

Responsibilities:

  • This role will be responsible for developing a strategic plan to expand and more effectively engage internal and external constituents to increase awareness of and participation in efforts to address Health Disparities (HD).

  • Works collaboratively with the internal teams to establish and oversee success metrics and performance monitoring criteria for addressing Health Disparities.

  • Advises leadership on required improvement to attain best in programs and recommends prioritization for operational enhancements.

  • Works collectively in a Matrix environment to develop initiatives to address Maternal Child health disparities and other clinical opportunities where health disparities are identified.

  • Responsible for content, program development and structure to support a focus on Health Disparities aligned to the CBU contracts.

  • Leads Partner discussions and conducts presentations as needed to support program needs.

  • Responsible for developing an enterprise wide education series on Health Disparity projects to increase awareness and knowledge on SDOH barriers to care.

  • Oversees the development of the Health Disparity resources and guidelines such Playbooks, program modifications, etc.

  • Develop and implement strategies to better involve and incorporate feedback from communities directly impacted by clinical engagement connected to Horizon's Health Equity programs and any health disparity campaigns.

  • Identify new and emerging policy trends, align advocacy priorities with community needs to better engage communities to address Health Disparities.

  • Develop Outreach strategies to enhance Horizon's status in the community. Serve as the lead voice as it relates to representing Horizon's commitment to addressing Health Disparities in New Jersey.

  • Continually evaluate operation of team/function to ensure on-going effectiveness and impact of clinical outreach and engagement efforts.

  • Ensure development and implementation of a clear strategy on development, production and dissemination of HD program materials which may enhance knowledge, understanding, and impact.

  • Liaise closely with the internal teams across HNS to ensure strategic use of current programs to enable and facilitate development of opportunity's to address health care disparities across key areas.

Education/Experience:

  • High School Diploma/GED required.

  • Bachelor degree in health or health care related preferred or relevant experience in lieu of degree.

  • Master's degree in a health or health care business related field such as MHA, MPH, LMSW or other relevant advance clinical degree preferred.

  • Minimum of 5 years' experience in the application of health equity and/or community health efforts; either in clinical operations, research, policy, or innovation.

  • Requires minimum 5 years' experience in developing and managing matrix relationships extensively to drive collaborative change and a motivated workforce.

  • Requires demonstrated ability to analyze complex information and data, and evaluate program performance and make program adjustments, is required.

  • Minimum 5 Years' experience in developing vision and strategy for community health initiatives, health equity and quality improvement and executing through successful program implementation.

  • Minimum 5 years successful experience in leading high-visibility process improvement initiatives from strategic design to evaluation across all lines of business

  • Requires demonstrated success engaging with communities of color and other at-risk populations.

  • Requires a minimum of 5 years' experience in one or more of the following settings: health plan, provider world, such as hospital, nursing home or other clinical setting with experience in leadership roles.

Knowledge:

  • Requires knowledge of clinical standards of care, preventive health, and Star measures.

  • Requires knowledge of health care delivery systems, distribution channels, health care industry and health insurance industry.

  • Knowledge of health plan industry practices, federal and state regulation, and accreditation standards.

  • Requires knowledge of strategic planning and development.

  • Ability to influence without authority.

  • Acumen in cross continuum provider delivery and health plan operations.

  • Refined oral and written communication skills.

  • High emotional intelligence and critical thinking ability.

  • Requires knowledge of NCQA, HEDIS, CAHPS, and QAPI for CMS.

Skills and Abilities:

  • Exceptional interpersonal and strong skills with the ability to build consensus. Must be able to interact and influence at the most senior levels of an organization and to work collaboratively across functions, levels and departments toward shared objectives.

  • Requires excellent verbal and communication skills.

  • Requires demonstrated aptitude for analytical thinking and ability to report findings accurately.

  • Requires strong interpersonal, organizational and problem solving skills

  • Ability to develop strategic plans to support company goals.

Travel:

  • Moderate travel required.

Salary Range:

$133,900 - $182,805

This compensation range is specific to the job level and takes into account the wide range of factors that are considered in making compensation decisions, including but not limited to: education, experience, licensure, certifications, geographic location, and internal equity. This range has been created in good faith based on information known to Horizon at the time of posting. Compensation decisions are dependent on the circumstances of each case.
